UK chemical end user certificates ?

I have noticed that certain chemical suppliers in the UK are asking for "End User Certificates" to accompany orders for various chemicals, for example Acetone and Diethyl Ether.

Can anybody enlighten me on the format of these certifcates and if they have to be issued from a goverment agency ?

A letter stating uses to which the product will be put, on business headed paper. It is an end-user declaration, rather than an end-user certificate.
