Hypo Method gone wrong...

A FOAF has attemted an eph. reduction using hypo/I and is now in hospital... Apparantly he was using a round bottomed flask connected with tubing to his condenser as his VCU and it exploded. Fumes go everywhere... you know the rest..
Does anyone have any tips on Vapor Containment and how to avoid this happening again?
